CCPHSD Community Health Assessment and Health Data

 Many of you are aware that the Health District recently conducted a Community Health Assessment.  As part of the assessment, the Health District completed a survey designed to allow community members to voice their opinions regarding local health issues, conducted community focus groups and presented a collection of quantitative data to help identify areas for health improvement in our communities.  The Assessment was recently updated and provides a wealth of information that may be useful to your Program.

The Assessment is accompanied by Community Health Data and Resources including a Community Dashboard which explores a variety of health and quality of life indicators.

The CHA will be followed by the Community Health Improvement Plan (CHIP), which will be available at the same link for your review later this month. The CHIP and the CHA will be used to develop a Strategic Plan that promotes the health and wellbeing of residents by intervening in the key processes that cause unintentional injury, preventing illness associated with poor lifestyle choices and pursuing affordable and accessible health care for all residents.
